Primary Health Care Provider Survey - 2012
Improving the health of families in Hastings and Prince Edward counties requires partnerships between families, primary health care and public health. At the Health Unit we have programs and services for families as well as connections to many community agencies. As primary health care providers, you are a trusted source of information for families. We want to hear from you about how much you know about public health and its services for families. And we want to help you to connect your patients to Health Unit resources. Your views will help to shape our community and primary health care outreach efforts. The Family Health department consists of the following programs: - Child Health,
- Healthy Babies Healthy Children (HBHC), and
- Reproductive Health.
Child Health
The goal of the Child Health program is to enable all children to attain and sustain optimal health and developmental potential. The Child Health program focuses in the areas of:
- Positive Parenting;
- Breastfeeding;
- Healthy Family Dynamics;
- Healthy Eating, Healthy Weights, and Physical Activity; and
- Growth and Development.
The Child Health program works with a variety of community partners to provide parenting programs, screening tools for parents to monitor their child's health and development; information and resources. The Child Health program works with community partners to create safe and supportive environments for children and their families.
Healthy Babies, Healthy Children
Healthy Babies, Healthy Children is a program for expectant parents and families with young children up to age 6. The focus for the program is on promoting healthy child development. The HBHC program provides new mothers with a telephone call from a public health nurse within 48 hours of hospital discharge, offers a postpartum home visit, and provides ongoing home visiting for parents of children up to 6 years of age.
Reproductive Health
The goal of the Reproductive Health program is to enable individuals and families to achieve optimal preconception health, experience a healthy pregnancy, have the healthiest newborn(s) possible, and be prepared for parenthood. The Reproductive Health program focuses in the areas of:
- Preconception health;
- Healthy pregnancies; and
- Preparation for parenting.
The Reproductive Health program works with a variety of community partners to provide prenatal classes, information and resources. The Reproductive Health program works with community partners to create safe and supportive environments to promote healthy pregnancies, healthy birth outcomes, and preparation for parenthood.
